    You are the heart and soul of this organization. You are the reason parents say,  "SPUT is the best thing since sliced bread" -- and they mean it whole-heartedly.

    There are two things we need completed well before you start working for St. Paul Urban Tennis this summer:
   
    (1), The form on the link below is required as you are paid as an independent contractor. No taxes will be taken out of your paycheck.Next January, you will receive a Form 1099, indicating how much you earned in 2009. You will need to report that to the IRS when you do your 2009 taxes
Please fill in the W9 form found at this link and mail it to us by May 15. Check the Individual/Sole proprietor box.
 
    (2) The City of Saint Paul and SPUT require that you have a background check, if you are over the age of 18, before you step on the court to teach.
